    Course Proposal Requirements

    Please be aware that once your Course Proposal Submission form has been reviewed, a member of Recreation Services Department will reach out to coordinate next steps. In addition to the submission form, you will be required to provide the following:

    1. Valid proof of insurance

    2. Appropriate tax documentation, including a W-9 form

    3. A signed contract, written and provided to you by the City of Prescott Recreation Services Department, outlining the specific terms, conditions and obligations related to your course offering.

  • ADA Compliance Requirements:
    The City requires all contractors, vendors, and service providers to comply with the Americans with Disabilities Act (ADA), including applicable provisions under Title II governing state and local governments.

    Vendors are responsible for ensuring that all services, programs, activities, deliverables, and materials provided under this agreement are accessible to individuals with disabilities and do not create barriers to participation. This requirement applies to all aspects of contracted work, including but not limited to physical spaces, communications, digital content, and public-facing services.

    For digital services (e.g., websites, applications, documents, or multimedia), vendors must ensure compliance with accessibility standards consistent with Web Content Accessibility Guidelines (WCAG) 2.1 Level AA, as required under current federal regulations.

    The City retains responsibility for ADA compliance in services delivered on its behalf; therefore, vendors must demonstrate the ability to meet these requirements and cooperate with the City to ensure ongoing compliance.

    Failure to meet ADA requirements may result in corrective action, up to and including termination of the agreement.

    • Undue burden means the cost or difficulty of making something accessible is very high compared to your organization’s resources and operations.
